           4. (U) TITLE: CASUALTY FEEDER CARDS

           5. (U) OBSERVATION: Casualty feeder cards were not completed to
           standard.

           6. (Ti) DISCUSSION: Many of the casualty feeder cards received
           at the division casualty cell were incomplete. They did not
           always contain information describing the incident where the
           soldier was injured or killed. They did not always contain
           information as to the disposition of the soldier (i.e., the
           location of the soldier and what wounds were received). This
           information is vital to the casualty reporting process.
           .Invariably the casualty call would have to contact the unit for
           more information in response to queries from corps or ARCENT.
           Units usually have to provide supplementary reports but did not
           always do so. Accuracy was not a standard. However, the
           argument most often heard from reporting units was that the
           reporting individuals did not provide or did not know much of
           the data mentioned above. Another argument was that the battle
           moved too quickly to provide accurate reports. The units merely
           attempted to provide a "quick and dirty" report to notify
           division of the casualties. Our success with casualty reporting
           was directly associated with the use of MOS OOR (Reenlistment
           NCO) soldiers who were assigned the special duty as patient
           affairs LNO. These soldiers performed duties in the corps
        hospitals in the division sector. They provided the much needed
           casualty information - status of patient, nature of the injury,
           extent of injury/procjnosis, NOK data. By SOP, the units had to
           provide an accurate personnel requirements report (PRR) in order
           to receive replacement personnel. The casualty report should be
           submitted in conjunction with the PRR, and must be as accurate.

           7. (U) LESSON LEARNED: Units did not accurately complete
           casualty feeder cards. Use of MOS OOR soldiers as patient
           affairs LNO was invaluable the success of casualty reporting.

           S. (U) RECOMMENDED ACTION: Include the casualty feeder card as
           part of common task training. Each individual soldier must know
           how to complete the casualty feeder card. Include casualty
           reporting in all FTX's and CPX'S. Continue use of unit liason
           NCO's with the hospitals to ensure accurate and timely reports.
