                              PRIVACY ACT ADVISEMENT


     AUTHORITY: 10 USC 3012

     PRINCIPAL PURPOSES: The purpose for soliciting this information is to
     conduct an investigation into the circumstances of an incident set
     forth in the letter appointing an AR 15-6 investigation.

     ROUTINE USES: Information gathered during the course of the
     investigation will be provided to the appointing authority for his or
     her determination as to what appropriate action if any should be taken
     against individuals who are found to have violated laws or regulations.
     It is also used for all other official purposes and may be shared with
     other state and federal agencies having an official need to know.

     MANDATORY OR VOLUNTARY NATURE OF DISCLOSURE: Unless you have been
     identified as a suspect of an offense under the UCMJ, you may be
     ordered to testify. Providing the information is mandatory. Failure
     to provide the information could result in disciplinary action under
     the UCMJ or Army regulations. if you are a suspect,providing the
     information is voluntary. There will be no adverse effect on you for
     not furnishing the information other than certain information might not
     otherwise be available to the comander for his or her decision in this
     matter.
