                  (2) Disease and nonbattle injury: Based on reports
             received, as of C + 78, the rate of hospital admissions for DNBI
             was 3.22/1000 soldiers/week. Outpatient DNBI rates were
             calculated as:

                      (3) The suspected cases of Sandfly Fever (PVNTMED
             SITRPT 29 SEP - 12 OCT 1990) were not confirmed by acute and
             convalescent sera. The Navy forward medical laboratory reported
             testing 15 Shigella isolates. Of the 15 tested, 12 were
             resistant to TCN and TMP-SMX. All were sensitive to
             ciprofloxacin.

                      (4) A case of visceral laishmaniasis was diagnosed in a
             30 year old active duty soldier assigned to the 82d Division in
             Saudi Arabia. He was evacuated from the theater on 23 October
             1990. Attempts are being made to interview the soldier to
             determine the likely source of the infection. This condition is
             transmitted by the Sandfly but is a much more serious medical
             problem than cutaneous leishmaniasis. This underscores the
             critical need to monitor and control insect vectors of
             life-threatening infectious disease. See Encl 3: Message,

             3. CURRENT SITUATION

                  a. Estimates:

                      (1) Heat casualty clusters reported: None.

                      (2) Water/food/vector disease outbreaks reported: None.

                  b. Resources available: CORPS level preventive medicine
ets have significantly increased. Preventive medicine
             support at some CONUS installations is short.

                  c. Preventive medicine course of action: Continue to
             closely monitor disease threat in the theater.
