IMPORTANT NOTE FOR PREVIOUS CALLERS TO DOD INCIDENT REPORTING LINE

IF YOU'VE MOVED OR TRANSFERRED OR IF YOU PREVIOUSLY LEFT A DSN (AUTOVON) NUMBER FOR A RETURN CALL--PLEASE CALL THE INCIDENT REPORTING LINE AGAIN TO PROVIDE A COMMERCIAL PHONE NUMBER

Attention all Gulf War Veterans, family members and others who called the DOD Incident Reporting Line in Monterey, California between May 30, 1995 and December 2, 1996: If you have moved or transferred since your initial call or if you left a DSN (was also previously called AUTOVON) phone number to receive a call back, please call the Incident Reporting Line again (1-800-472-6719) as soon as possible and provide a commercial phone number at which you can be reached. On January 8, 1997, representatives from the office of the Special Assistant to the Deputy Secretary of Defense for Gulf War Illnesses call back team, whose mission includes calling back every veteran, family member or concerned person who has called the Incident Reporting Line, began contacting previous callers who had called between May 30, 1995 to December 2, 1996. In making their initial followup contacts, team members found that some previous callers had either moved or transferred or had left DSN phone numbers where they could be reached. The call back team currently does not have a DSN phone capability and needs commercial phone information to contact previous callers. Thus, if you have moved or transferred since your first call, or left a DSN phone number where you could be contacted, your help is requested in calling the Incident Reporting Line again at 1-800-472-6719 as soon as possible and providing a current commercial phone number where you can be reached. Additionally, it would be very helpful if you could also provide the best time (s) and days-of-the-week during the work week (M-F) to contact you. Your assistance is greatly appreciated and will help in getting previous callers contacted more quickly.
