                     2. Did the unit order the following
                     contingency supplies on the DA Form 2765-1?

                          a. CPOG (by size) (one per individual)

                          b. CP Footwear covers (one per individual)

                          c. CP Gloves (by size) (one per individual)

                          d. Batteries, BA 3517/u

                          f. Mark I Kits (three per individual)

                          g. Did the unit request shortage equipment
                          from the supporting unit?
