SAUDI ARABIA
increased to 305 cases in 1988, all in the southwestcm provinces of Asir and Jiznn. No animal
reservoir for either I- tropics or L. dotiovaiii has been demonstrated In Saudi Arabia.
SCITISTOSOM@S (2 to 6 weeks)
Trnasmiss@ector Ecology: Penetration of the skin by waterborne larval forms (ccmadae) that
develop in snails in freshwater impoundmcnts.Dic primary Intermediate host for Schistosotiia
mansoni (intestinal) is Bioniplialaria arabica and for S. liaentaloblum (urinary) Is Ditlitnits
bcccarii.
Risk Per@istributton: Transmission occurs year-round, with risk elevated during the spring
rainy season, March through May. Distribution is focal In the western (in wadis nnd cisterns)
and central (in oascs) provinces. Intestinal sch@ccoralasis foci occur In the central 0 loll, Riyadh),
northern (Al Jaw[), northwestern (Tabuk, Medina), Midwestern (Makkah, Al Doliali), and the
highlands of Uic Southwestern provinces (Asir, Najmn). Urinary schistosorniasis foci occur In
the lowlands of the Southwestern (Jizan) and Midwestern (Makkah) provinces.
Rcmnrks- Although still prevalent (overall infection rate estimated at 2 percent or 20,000 cases) In
the 12 identified foci, the annual reported incidence O= than 1,000 clinical cases) was decreasing
during the late 19M because of the cffccdvcnm of control programs.
UNIER DISEASES ENDEMIC IN TIIZ INDIGLi'NOUS POI'ULATION
Zoonotic d - Brucellosis (cnzootic, pardcularly.in go-,iLs; human cases caused byu. melliensis
n the summer;
usually due to consumption of raw gott milk); ectilnococcosis Otydadd d@so; occurs In an
estimated 15 pc=nt of stray dogs; clinical cases In liumans relatively uncommon, but accounts
for many surgical procedures); rabies Qittman cases very sporadic, usually in die northern or
eastern rum[ areas; foxcs constitute the primary cnzootic reservoir, with some spillover Into the
stray dog population); nnthrax (very sporadic in rural arm during summer monl@, usually
occupationally related to livestock tcaring); and Q fcvcr (rarely reported In liumans but enzoodc
in livestock, and human scrologics in rural arcis indiratc exposure).
Vectorl>orne d@s - Mtirine typhus (nea-borne, occurs very sporadically), oncliocerclasls (black
fly vectored, could occur in the extreme souiliwcs4 may no longer be endemic); plitgxto (sylvauc,
tlc.a-bornc, last reported near the Saudi Ambin-Ycmcn border fit 1969, may no longer be endemic).
Sexually transuaitted diseases - AIDS (no officially reported cises through the cud of 1989, although
some arc known to have occurred; in-country testing for IIIV infection reportedly Is perforated,
but no data arc available). -
Other Infectious d -Trachoma (highly endemic, especially in rural arm; sit estimatcd 6 per
cent of the population have active infections, but control measures reportedly reduced incidence
during (tic 1980s-, remains tire most common cause of blindness in Saudi Arabia); soft frnnsrttitted
helminthic infedlons (including roundworms, hookworms, and wliipwornis; common In rural
areas, infection rates reportedly arc low In die central and =tcm provinces); nonvenereal endemic
eas, infcc@oq rates up to 20 percent amongsome nomadic tribesmen);
and tuberculosis (cnd@c, annual incidence reportedly was declining during tire late 1980s, but
prevalence remained @gt4 especially in rural areas).
