ANNEX N (PERSONNEL SERVICE SUPPORT) TO 1AD OPLAN 65-91M (MOVEMENT TO FAA
GARCIA)
(a) Witnesses will report casualties on a DA Form 1155
(Witness Statement) and on a DA Form 1156 (Casualty Feeder Report).
Forward this information to the Personnel Action Center (PAC), where a
Casualty Report is initiated an TACCS. A good technique is to issue one
card to each soldier. If DA Form 1156s are not available, eyewitness
should write down legibly on any piece of paper the name, grade, SSN,
unit, location of incident, time, and circumstances.
(b) The eyewitness, first line supervisor will verify the
Casualty Feeder Report information for accuracy and pass the report to the
Dn PAC. PAC will verify and add any available information to the report,
Such as hospital status, and forward it to the 400th PSC (normally through
the Brigade S-1 and Division GI/AG). I+ the Bn PAC has TACCS capability,
they should enter Casualty Feeder in-Formation into their TACCS and send
automated Casualty Feeder Reports to the PSC. TACCS automated Casualty
Feeder Reports are preferred, but manually prepared DA Form 1156 Cards are
acceptable..
(4) Religious Support-
(a) Priority of DS/GS Religious Support prior to hostilities
is to unit religious services, sacrament and ordinances Protestant
services in all Task Forces will be conducted to Company/Battery level;
Catholic services will be conducted at least to Battalion level.
(b) Priority of I)S/GS Religious Support after beginning a-F
hostilities will be to casualties, then to units not currently engaged.
(c) All MSC Chaplains be prepared to release up to one (1)
UMT to support regeneration/reconstitution efforts under Division control.
(d) UMT's will report reported/suspected violators a-F the
Law of War to the Commander, then to next higher HQ in technical channels.
(e) Each UMT will carry sufficient ecclesiastical supplies
-For at least 21 days of operations. Limited re-supply will be carried by
DISCOM and FSB Chaplains. Cross level ecclesiastical supplies as
necessary.
(-F) Report all UMT casualties through SI/GI Channels to
Division Chaplains.
(g) Catholic UMT w/in combat Brigades will cover Brigade
Task forces, and adjacent supporting units. MSC Chaplains have authority
to reposition assets in the event of Task Force casualties. Request
replacements from the Division Chaplains. Division will cover remaining
forward units. D-REAR and rear elements will be supported by Catholic UMT
in 7th Support Group.
