SUBJECT: DOCUMENTATION GUIDELINES FOR ANTHRAX IMMUNIZATION
1. Soldiers in the Southwest Asia Theater of operations during the time
frame I January through 16 February 1991 were ordered to receive the
anthrax immunization which was the only "secret" immunization given in
theater. Some soldiers received one immunization; others received a
second injection (Al, A2). During this period of time, Operational
security precluded annotating this immunization in the individuals shot
record and/or the medical record. This guidance has since been

guidelines should be followed for documenting this activity:
a. Unit clerks or a unit representative selected by the individual
commander should prepare an alpha roster of personnel in the theater
during the specified periods of time.
b. Many of the immunization sites have since redeployed and their
records and the roster of units they immunized are not available. For
that reason, it must be assumed that all service members in SWA during
the specified time period received one or two anthrax injections. Due to
the discomforts associated with the injections, soldiers will most likely
remember whether they received a single or multiple vaccination. The
soldiers, word will be accepted, and one of the attached slips will be
stapled to the medical record and/or the shot record upon redeployment to
home station. (See attachments 1 and 2)

Unit clerks at the time of immunization provided alpha rosters which were
validated and returned to the unit. Units are authorized to annotate the
shot records of deployed soldiers with the appropriate immunization
record.
3. To emphasize: no theater records of the anthrax vaccine program are
available. Each unit deployed to SWA during the specified time frame
should survey their soldiers and annotate the individual shot records
with the appropriate form.
