          4. (U) TITLE-. OPERATIONS: MEDICAL: OPTOMETRY SCREENING


          S. (U) OBSERVATION: Requirements for optometry screening were
          not provided to unit commanders.

          6. (U) DISCUSSION: The individual soldier did not receive
          procedural information for a smooth transition through the
          optometry clinic:; eg, soldiers not directed to bring medical
          records nor provided clinic location.

          7. (U) LESSON LEARNED: Unit commanders must provide detailed
          information for optometric requirements.

          e. (U) RECOMMENDED ACTION: Schedule optometry screening earlier
          in the synchronization matrix to ensure receipt of eye wear
          prior to Ltnit's deployment.
