JOINT UNIFORM LESSONS LEARNED SYSTEM (JULLS)
OBSERVATION WORKSHEETS
1. General Info Please use the form provided. Hand written (printed)
forms are accep able provided they are legible. Acronyms should be spelled
.out for the first appearance. Each worksheet must deal with only one issue.
If the observation/problem/issue is Fort Riley specific and should not be
forwarded to HQ 5A or to FORSCOM, write "Local Issue" at the top right on
each side of the worksheet.
2. Title. Start 4ith the functional area for the issue (such as I-og-@st4ics
or PYrs-onnel), and follow with a short description of the issue.

4. Date: The date you prepared the worksheet. Use MM-OD-YY format.

7. Rank/Name: The rank.or title and name of the preparers "Mr" is OK.
8. AV #/COMM: The preparer's AUTOVON and Commercial phone numbers.
9. Classif: Insert the letter of the worksheet's classification when it
is filled out. Go ahead and write or stamp the same classi-
fication at the top and bottom of both sides in the blanks
provided.
