SUBJECT,. Encounters with Combat Engineers Performing EOD-related Tasks

             F. Insufficient priming charges were used for the destruction of ammunition.
        [Photos #6, #7 and #81

             G. Improperly placed priming charges would have caused incomplete destruction
of the ammunition in the bunkers. [Photos #9 and #101

It. Numerous det cord branch lines crossed other branch lines; many of the
branch lines had kinks or sharp-angled bends. [Photos #11, #12 and #131 Either
practice can cause the det cord to sever itself at that point.

             I. Numerous US M15 antitank land mines were fuzed and armed. These mines were
apparently to be used as demo material, as they were placed under stacks of
ammunition. Stacking heavy material on top of armed landmines could cause the mines
        to function as designed.

             J. Bulk US C-4 M112 blocks were found scattered at the bunker entrances. No
priming of these explosives was attempted. [Photos #14, #15 and #161

             K. Numerous loose non-electric blasting caps were scattered on the floors of
the bunkers. Unattended primary explosive devices could cause catastrophic results.

        4. Because of the haphazard demolition procedures utilized, the 146th FOD was
required to disassemble the poorly constructed demolition set-ups, then reconstruct
the demolition set-ups to insure complete destruction of the enemy ASP.

        5. POC this command is MSG Gjerning, 011-966-3-895-0602, ext. 3129 or 3127.
