              4. (U) TITLE,. OPERATIONS: MEDICAL-. OPTOMETRY SCREENING

              5. (U) OBSERVATION: Requirements -For optometry screening were
              not provided to unit commanders.

              6. (U) DISCUSSION: The individual soldier did not receive
              procedural information for a smooth transition through the
              optometry clinic; eg, soldiers not directed to bring medical
              records nor provided clinic location.

              7. (U) LESSON LEARNED: Unit commanders must provide detailed
              in-Formation for optometric requirements.

              S. (U) RECOMMENDED ACTION: Schedule optometry screening earlier
              in the synchronization matrix to ensure receipt a+ eye wear
              prior to unit's deployment.
