             personal. priorities for impending combat.                        All decisions riade
             during this phase were naturally affected by the exooctation of
             entering combat operations a-, any -Iii,-,e.               Specific areas involving
             pr;-,-,iarily human factors included:

                      (1)    Night ODerations.           Nigh", flight operations wore c;-tod
             as the "dumbest" t@irg we did in SWA by a4---crews surveyed after
             the operation.           Survey results are rt"lached as encl 2.                       When
             combined wit@ material and environmental orobleirs associated with
             this task it became the squadrons h,-ghe;-, risk -n,'-ssion.                          Human
             factors were the prilary cause of the OH-58 Class C mishap 24
             Dece-,ibar.      They were a contributing factor in the crash of All-
             1 on 7 February.                Both accidents occurred during hours of
             dankness.           Veh;-cl.e opera"-ions at night were also extremely
             difficult.          Vehicle crows could easily become misorientated for
             long periods        of title while driving at nigh-,.

                      (2) Low level/iii-gh Speed Flight. Near continuous flight in
             the "avoid" area of heigh@,               velocity diagrams was required for
             survivability.                     Human       limitations            of         emergency
     ....-...recognition/reaction            time     combined       with      inherent       aircraft
       z-:---@-.responses made    flight in this       mode@ constant        high   risk.
