armed hostile force and the ultimate sense of mission urgency did
however, naturally increase overall risk.
6. REDEPLOYMENT PFASE,. includes the period from final departure
from Iraq (7 April) until 24 April when most soldiers in the
squadrork.returned to Germany. Return of all unit equipment is not
complete as of this writing. Observations to date are however
noted below. Reported incidents during the redeployment phase:
DATE UNIT EVENT
7 Apr A HEMETT (HT 108) collides w/stopped trailer
15 Apr A POV ran into rear of stopped HEt4ETT - No US damage
17 Apr p *OH-58D (723) elec fire in flt - Class E Air
a. Human Factors. Human factors from deployment phase were
also present during redeployment. Direct involvement of unit
personnel in ship/rail load operations has decreased compared to
deployment phase. Sports activities/injuries remained low while
the unit was in Saudi Arabia during this phase. Injuries have
returned to pre-deployment level (2 recordable of 8 reported)
since soldiers returned to garrison in Germany.
b. Environmental Factors. Initial redeployment tasking
required vehicle convoy operations over long distances of a tank
trail network in southern Iraq. Heavy vehicle traffic on these
ery fine
dust which severely restricted driver vision. Two rear end
collisions occurred due to dust reduced visibility on these tank
trails.
C. Material Factors. No new material factors were
.encountered during redeployment. Intensity of operations during
previous phases did however, deteriorate the condition of most
equipment. Vehicles returned to home station for example are in
generally poor condition. To date many have arrived with
mechanical damage and up to half must be jump started or towed off
railcars. Disabled vehicles complicate the rail off load by
requiring tow vehicles to be backed down a long line of rail cars.
d. Lessons Learned. With exceptions noted above, no new
problems occurred during redeployment. Strong first line
supervisor leadership remained the single most important factor in
risk reduction for port, rail and staging area accident prevention
