            SUBJECT: Leishmaniasis
                                                @v I
            1. Cutaneous leishmaniasis (Baghdad Sore) is  an ulcerating
            lesion of the skin, caused by the bite of an  infected sandfly.
            It begins with an inflammatory papule that slowly increases in
            size and ulcerates. The base will crust over but the ulcer
            spreads under the edge of a firm and raised border. The lesions


            are usually on exposed skin and are rarely seen 4-n the scalp, on


            the palms or soles. It is most common in central and eastern
            Saudi Arabia, Southern Iraq and along the Tigris River.


            2. The disease will not have an immediate impact on tie combat
            power but will cause disfigurement and loss of the soldier to the


            medical channels.


            3. There '-s no vacc'no available against leishmaniasi-s.
            Personal protective measures, such as the use of insect repellent


            on exposed skin, are very effective in preventing disease.
            Although the mesh size is large enough for sandflies to
            penetrate, mosquito netting sprayed with permethrin can provide
            protection. Vector control can also be achieved by spraying
            indoors wi@h pyrethrum. Strict adherence to sanitation control
            measures should be followed to limit resting sites. Insecticide


            spraying of outdoor breeding sites by preventive medicine
            personnel can be accomplished with chlorpyrifos. Do not use on
            tents as it will remove the water repellency of the fabric.


            4. Unit field sanitation teams are a valuable local asset and
            should be utilized to help ensure that proper sanitary measures
            are being employed. Additional division and corps preventive
            medicine units are available for assistance.
