            anyone without a documented polio booster as an adult should
            receive one dose of either OPV or IPV. Cholera immunizations are
            not routinely recommended. IsG is not routinely recommended.

            6. Malaria

                Category A. High risk. Risk dependent on the area and
            season. Chemoprophylaxis wi"h Chlaroquine and terminal
            i;hemoprophylaxis with Chloroquine/Primaguine is generally
            recommended for routine brief urban port visits to risk areas
            during risk periods, as well as personal protective measures.
            Consult NEPHU-7 for other types of visits. A summary of risk and
            -chemoprophylaxis follows:

                  Pakistan: Moderate  to high  risk. Chemoprophylaxis
            recommended for all port visits regardless of duration of stay.
            Chloroquine resistance reported.
                - Iran: Areas of risk are rural areas only in the pro@inces
            of Sistan-Baluchostan and Hormozgan, the southern parts of Fars,
            Kohgiluyeh-Boyar, Lorestan and Chahar Mahal-Bakhtiani and the
            north of Khuzestan. (Low to moderate risk in Karachi).

                  Saudi Arabia: Risk is year round, greatest along the Red
            Sea Coast. Risk occurs in all areas in the western provinces
            except the high altitude areas of Asir Province (Yemen border),
            and the urban areas of Jeddah, Mecca and Medina and Taif.

                 Category B. Low to moderate risk. Chemoprophylaxis
            recommended in areas of risk. No Chloroquine resistance
            reported.

              - United Arab Emirates: All areas at risk in Northern
            Emirates, except no risk in cities of Dubai, Sharjah, Ajman, umm
            Al Qaiwan and Emirate of Abu Dahbi.

                Category C. Low risk. Chloroquine/Primaquine prophylaxis
            recommended in areas of risk in addition to personal protection.
    No Chloroquine resistance reported.

                - Iraq: Risk areas include all areas in northern region
            below 1500m:  @ok, Erbil, Minawa, Sulaimaniya, Tamin Provinces.
            Risk is from May to November.

                   Oman: All areas are at risk all year round.
