                                         EXECUTIVE SUMKARY



                  Operation Desert Shield/Storm provides an opportunity to examine
             the Navy Medical Department's implementation of the total-force concept
             under operational conditions. The Navy's requirement for medical
             Selected Reserve (SEIRES) personnel increased in three phases. During
             the initial phase, from August until early November 1990, the Navy
             recalled SELRES personnel to replace approximately 84 percent of the
             active-duty personnel who deployed from medical and dental treatment
             facilities (MTFs and DTFS) in the continental United States (CONUS).
             During the second phase, from November until mid January, SELRES per-
             sonnel continued to backfill as more active-duty personnel deployed.
             Overall replacement levels fell slightly to 80 percent. In addition,
             SELRES personnel completed staffing of the hospital ships and staffed
             two fleet hospitals. The third phase of the recall, to expand CONUS
             facilities to care for casualties w a in progress when hostilities
             ended, and is documented separateiy.1

                  Experience from Desert Shield/Storm helped to identify a number of
             problems with implementing a limited reserve recall. During this opera-
             tion, the process of matching reserve resources to requirements was
             ongoing. In addition, for medical reserves this process took place on
             an individual level. MTFs had no systematic method of developing and
             revising requirements as their mission evolved. The Navy initially
             lacked ready access to the information needed to identify individuals
             for recall and to track the status of the recall. These problems
             contributed to the lag in replacing deployed active-duty personnel at
need to deploy some active-duty personnel on
             short notice to meet shortfalls at the fleet hospitals staffed with
             SELRES personnel.

                  Operation Desert Shield/Storm also offers some lessons on the
             appropriate structure of SELRES units designated for backfill of CONUS
             facilities. These units have no nonmedical personnel to replace those
             that deploy from CONUS medical and dental facilities. In addition,
             reserve staffing for the hospital ships came from these units and
             individual SEIRES personnel lacked shipboard and orientation training.
             The Navy should consider designating SELRES forces for these missions.
