              INTRODUCTION


                   Operation Desert Shield/Storm provides an opportunity to examine
              the Navy Medical Department's implementation of the total-force concept
              under operational conditions. The contributions of reserve forces are
              clearly essential to meeting the requirements of wartime medicine.
              Medical Selected Reserve (SEIRES) personnel replaced active-duty per-
              sonnel at medical and dental treatment facilities (MTFs and DTFS) in the
              continental United States (CONUS), maintaining services and preparing
              the facilities to receive casualties. Other SELRES personnel were
              deployed to the Gulf to meet theater medical requirements.

                   The Navy's SELRES is the principal source of trained units and
              personnel to augment the active forces. Under Title 10 of the United
              States Code (U.S.C.), section 673b, the President may call up to 200,000
              SELRES personnel for 90 days to support operational requirements without
              declaring a state of war or a national emergency. This statute was in-
              tended to strengthen the total-force concept, which places considerable
              resources and capabilities with the reserves by increasing accessibility
              to the reserve forces. The first use of the authority under title 10
              U.S.C. 673b took place on 22 August 1990 in support of Operation Desert
              Shield.

                   This research memorandum begins with an overview of Navy medical
              SEIRES employment in support of Operation Desert Shield through
              mid January. It focuses on the development of requirements for recall
              of medical SELRES personnel and the implementation of the recall.
              Appendix A contains background information on the organization, mission,
 contains a chronology
              of the Navy medical SELRES recall. Additional analyses of the reserve
              recall through the end of Desert Storm will be documented separately.


              OVERVIEW


                   Approximately two weeks after the Iraqi invasion of Kuwait, the
              Secretary of Defense (SECDEF) requested that the military departments
              and the Joint Chiefs of Staff (JCS) prepare a plan for the call-up of
              reserve forces in support of Operation Desert Shield. On 22 August, the
              President authorized the recall of SELRES personnel under Title 10
              U.S.C., section 673b. After reviewing the estimated requirements for
              each service, SECDEF gave each service secretary the authority to recall
              reserves up to a ceiling. The service secretaries reviewed and autho-
              rized the recall of specific reserve units. An overview of the reserve
              recall for the Navy and Marine Corps in Desert Shield may be found in


                   SECDEF initially authorized the recall of 6,300 Navy SELRES
              personnel. The Chief of Naval Operations (CNO) subsequently approved
              the recall of up to 2,380 medical personnel. Subsequent requirements
              resulted in higher SE@S authorizations, as summarized in table 1, and
              the recall of additional medical SEIRES personnel.
