individuals and units to meet the requirement. BUMED assumed this
responsibility because computerized personnel records with COMNAVRESFOR
lacked the information needed to identify qualified medical SELRES
personnel. After resolving this data problem, the responsibility for
nominating units for recall returned to COMNAVRESFOR.
The decision to deploy medical SELRES personnel followed CINCCENT
tasking to provide an additional 1,000 echelon-III beds and bring the
hospital ships to 1,000-bed staffing levels. OP-093, OP-01, and OP@06
decided to use SELRES personnel to meet these requirements. This option
would not' have been practical under the initial 90-day service limit for
recalled SELRES personnel; however, this limit was extended to 180 days
in November and subsequently to 360 days following authorization to
recall the Individual Ready Reserve (IRR) on 18 January.
CONUS NTF Requirements
Determining the requirement for medical personnel at CONUS 14TFs
proved to be a challenging task during Operation Desert Shield. Plans
for the use of Program-32 assets were developed with the assumption that
the start of deployment operations would lead to full mobilization. In
the absence of full mobilization, the mission of the CONUS MTFs during
Desert Shield was not immediately clear. As that mission evolved over
time, the appropriate number and mix of providers also changed. The
NTFs had to identify their evolving requirements for SELRES personnel
while faced with rapid changes in staffing levels as active-duty per-
sonnel deployed and the SEIRES personnel were recalled to replace them.
Under the plan for transition to full mobilization, CONUS MTFs
shift from peacetime care to casualty receiving within 30 days. During
the transition, active-duty medical personnel deploy to staff units and
medical facilities in theater. As active-duty personnel deploy, CONUS
medical facilities restrict services to providing essential care for
active-duty personnel and to dependents in isolated areas. Other cate-
gories of personnel receive care in the civilian sector under the
Civilian Health and Medical Program of the Uniformed Services (C@S).
SEIRES units drawn from Program 32 are recalled to replace deployed
active-duty personnel at CONUS facilities. They provide care to return-
ing combat casualties and support the increased operating tempo of Navy
and Marine Corps activities. In addition, Program-32 SELRES personnel
serve as the replacement and rotation base for deployed active-duty
medical personnel. Full mobilization was not required for Desert
Shield/Storm.
Shortly after the Iraqi invasion of Kuwait, BUMED activated the
Medical Personnel Unit Augmentation System (MPUAS) to meet immediate
theater requirements for medical personnel. MPUAS identifies active-
duty personnel assigned to CONUS MTFs and DTFs to staff or augment
deployable medical platforms and units such as hospital ships, fleet
hospitals, and Marine medical companies, as described in (2). However,
