      SUBJECT: (U) Side Effects of Pyridostigmine Bromide (PB) Nerve
      Agent Pretreatment

      NARRATIVE:

           1. (U) Potential side effects of pyridostigmine bromide are
      listed in F74 8-285. Many questions have been raised by the field
      on the severity of these side effects and whether they impact on
      soldiers and aviators.

           2.       The flight surgeon of the 160th Special operations
      Unit has issued guidance that none of his pilots will take
      pyridostigmine even in a chemical environment because of potential
      performance decrement of taking the nerve agent pretreatment. MSG
      Murphy, AV 635-9422. The chemical NCO, has requested OTSG guidance
      on issue. The unit departs on Monday, 13 Aug 90.

           3. (U) Current USAF policy permits their pilots to take the
      PB in a chemical environment. Initially, pilots are given one PB
 *pill to assess idiosyncratic reactions. If no reactions, then they
      are permitted to take the PB. Col Pate in the TAC Surgeon's Office
      is POC for the policy, AV 574-4610.

      TASK:

           1.       Assess the risk of the PB regimen on pilot performance
      and respond to MSG Murphy ASAP. He needs a hard copy of the OTSG
      position on PB.

           2. (U) Send out message to the world describing the risk or
      lack of it in taking PB.
