            S. (U) OBSERVATION: Documentation of care on appropriate forms
            was a problem.

            6. (U) DISCUSSION: The JSNAG (Joint Services Nursing Advisory
            Group) had developed standard forms to be used for documentation
            by all services. However, implementation had not been
            executed prior to the start of Desert Shield. There needs
            to be a means of patient identification for the forms. (i.e.
            stamp plate and an addressogragh style machine.) Othe@se, much
            time and energy is spent @iting. Not only does this use
            precious time, it also creates the potential for error and
            illegible records.





            7. (U) LESSON LEARNED: Implementation of JSKAG guidelines in a
            timely manner. Develop a system to use a imprinted card or
            device for identification of patient forms.

            8. (U) RECOWMENDED ACTION: Implement JSNAG guidelines and
            develop a @em to utilized an imprinted card for
            identification of patient forms.
