antibatica war* discontinued, 3. Doxycyclina and ciprotloxacin
may have offered some advantage over penicillin in this
(inhalation anthrax) setting, Ind 4. combined antibiotics and
vaccination in the post- @ asurs period resulted in the beat

            survival.

a simplified review of pathogenesis coupled with speculation
about mechanism of antibiotic activity might shod light on the
observed findings. Anthrax organisms are most effectively

delivered by i@lation in the form of sports. Inhalation of
these spores results in savdinq of terminal rtapiratory
bronchioles. Spores than are phaqooytosed by alvsolar
macrophages and transported to lymphatic organs in the chest
(pulmonary and madiastinal lymph nodes) and elsewhere. There,
the spores germinate, becoming "vegetative'' organisms which
release toxins and exert adverse physiologic *ffoots. With
continued replication of these "vegetative'' baoillit increasingly
larger numbers of organisms and their toxins circlalats. 'Finally,
the host is overwhelmed and de&= ansues. with administration of

appropriate antibiotics, vegetative forms (= spores) are
killed. In some cases, this may be adequate for a "cure.,, In
many others, however, additional spores probably remain dormant
until antibiotics are discontinued. In these individuals,
residual spores then garminate, and the r&plioative process
begins anaw. Thsoratically, these persons could remain on
antibiotic s for prolonged periods, and survive. In practice,
