          Against Biological Warfare (BW) Agents

          toxoid (unquote), as.appropriate. The   transcription from
          temporary medical records or unit rosters can be accomplished by
          either supporting medical personnel or, in the case of RC units
          with no direct medical support, by the personnel records section.

          "5. For those unit   which have rosters recording some or all of,.
          these vaccinations, DA has requested that copies of such rosters
          be forwarded to them-as a backup. No new'reports should be
          generated to meet this request. Units should retain the
          original roster for their records and pass a photocopy up the
          chain of command to FORSCOM Suraeon, ATTN: FCMD-PM, Ft
          McPherson, GA 30330-6000, NLT 36 Sep 91. FORSCOM will compile
          the rosters and forward to-bffice 'of the surgeon general."

          2. It is critical that responsible commanders ensure that the
          date and type of vaccination is legibly annotated in the     I
          individuals health record, SF 601 (immunization Record) and the
          individuals PHS 731 (Yellow Shot Record), IAW AR 40-562.

          3. A photocopy of any unit rosters recording any or all of
          these vaccinations will be forwarded to this headquarters,
          ATTN: AFKA-PR-R, not later than 16 S ptembet 1991.
