           MEMORANDUM FgR COMMANDER, MAJOR U.S.  ARMY RESERVE COMMANDS
           SUBJECT: Medical Records and Rosters Related to vaccination
           Against Biological.Warfare (BW) Agents


           1. The following  message, CDR, FORSCOM,  FC31, 191755Z  Jun 91,
           SAB, is quoted for immediate action:
           111. This message directs specific actions be taken to    properly
           document all soldiers who received special vaccinations against
           SW agents during Operation DESERT STORM.
           "2. During the period of 11 3an 91 thru 28 Feb 91, some
           designated units and individuals received immunizations against
           anthrax and/or botulism. Each vaccine was administered as an
           initial series of two injections,.about two weeks apart.
           Because of the classified'nature of the vaccination program, and
           various interpretations-of the degree of security required,
           medical personnel recorded receipt of.the vaccines by different
           methods and on various forms. If no temporary medicalrecords
           or forms were available at the immunization site, units recorded
           receipt on a unit or sign-in roster.
           "3. For anthrax vaccine, medical personnel may have recorded
           the shot as (quote) anthrax, a vaccination, a-vacc, a-vax
           (unquote) or something similar. For botulinum toxoid, medical
           personnel may have recorded the shot as (quote) botulinum,
           bat-toxi b vaccination, b-vac :c, b-vax (unquote) or something
ilar.

           114. Classification of the SW immunization program associated
           with DESERT STORM was downgraded on 8 Apr 91. All AC and RC
           units must now ensure that the date and type of vaccination is
           clearly annotated in the individuals health record on SF 601
           (Immunization Record) and on the individuals PHS 731 (Yellow
           Shot Record), if available, IAW AR 40-562. This annotation
