           SUBJECR: Clinical Consultant for Chemical Casualty Care's Annual Report to The Surgeon
           General for 1990



           1. Chemical casualty care became a topic of emphasis for the medical support of
           Operation Desert Stonn/Desert Shield. From September to December 1990, the Army
           Medical Department achieved a high state of readiness for chen-dcal casualty care, thanks
           to the interest and strong support of commanders at all levels, and to the efforts of about
           30 Medical, Veterinary, Medical Service and Nurse Corps officers from USAMRICD,
           USAMMDA, TSG's staff, FORSCOM, and the Academy of Health Sciences in education,
           doctrine and new product fielding. I am especially appreciative for MA.J Roger McfntosWs
           service as Acting Consultant from September to December 1990.

           2. A forward detachment of USAMRICD operated in the U.S. Central Command area
           of responsibility from September 1990 through March 1991. Activities were briefed to
           TSG in December 1990, as shown at TAB A. Medical Management of Chemical
           Casualties Course accomplishments in theater are shown at TAB B, and later operations
           of an expanded USAMRICD Forward Detachment during Operation Desert Storm are
           detailed at TAB C, along with a report of the only clinically confirmed U.S. chemical
           warfare agent exposure, shown at TAB D.

           3. The major purpose of this report is to request TSG's support and tasking of
ion issues
           generated by Desert SMeld/Desert Storm, as shown at TAB E. These concern doctrine
           and training, postgraduate medical education, a unit network of subject matter experts,
           and a response mechanism for USAMRDC scientific support of unified command missions.

           4. Doctrine and training remains a crucial issue in medical chemical defense readiness,
           as discussed in Lesson Learned 1 at TAB E. It seems clear that there is a tendency to
           tie doctrine and training to the fielding of new equipment that lacks credibility with
