D. Chemical Warfare Decontamination and Equipment Construction:

 PROBLEM: There was little or no guidance on a base decontamination plan.
 Coordinated with Disaster preparedness on a plan but they
 didn't seem to have one established. There was no
 decontamination Equipment available. It took us some time to
 build and acquire support equipment for an open air
 decontamination system.

 SOLUTION: Decontamination equipment and support equipment should be part
 of the units mobility pallet. Units need to be prepared for
 chemical decontamination upon arrival . Disaster Preparedness
 also needs to arrive at a location with a chemical
 decontamination plan. This will ensure proper coordination
 and set-up at the decon site.

 E. Helmets not modified for use with the CBO mask IAW to 14P3-1-131.

 PROBLEM: Several crewmembers were using custom fitted lines that are
 not modified for use with the C80 mask. Crewmembers were
 experiencing hot spots in the ear and crown area.
 SOLUTION ON: Replace Custom fit liner with Thereto Plastic Liner (TPL)
 ASAP. If TPL is not available modify custom fit liner IAW applicable E
 directives.
 F. Aircrew chemical Ensemble replacement at the FOB.
 PROBLEM: Air Crews ware issued full BOI on D-Tags. They were
 responsible for carrying the D-bag to the FOB. Upon arrival
 at the FOB, crews were tasked to keep one set of ACDE and
 the rest were planed over to life support for storage. Due to the limit
 storage area, all ACDE assets were stored in a mobility
 conex. Several aircrews used the ACDE and sometimes needed
 replacement. This process was very time consuming.
 SOLUTION: Crews should only be issued one set of gear for flying to the
 FOB and all missions thereafter. The rest of the gear should
 be shipped to the FOB for proper Accountability and follow on issuance
 G. Issuing survival vest to Dedicated Crew Chief and Incentive fliers.
 PROBLEM: DCCs are not trained in the use of survival vest and
 compontents. -
 SOLUTION: Include the DCC in Life Support equipment and survival
 training.
 H. Survival Knife Sheath Location.
 PROBLEM: Knife sheath is located too low. Sheath sits on individuals
 hip causing irritation and discomfort in prolonged sitting
 position.
