File: 970207_aadci_086.txtODS MEDICAL LESSONS LEARNED C3I and TRAINING CATEGORIES C3I ISSUE NO. l9 Coding of Clinical Records: Training Needed (SGHR ITEM NO. 130) DISCUSSION: Initially, AFOMS/SGSB directed coding of inpatient records of 3Es and 4Es. Deployed personnel lacked coding skills' reference materiel, and automation. WARMED Instruction Handbook indicates 3Es and 4Es will ship records and loose documentation to a central repository in CONUS for coding, storage, and biometric accountability. Upon coordination with AFOMS/SGSB a compromise was reached where the AF Form 560 would be forwarded to AFOMS for coding and biometric purposes. ICD-9 coding books were obtained for CONHOSPs to support reporting. RECOMMENDATIONS: AFOMS/SGSB propose a change to WARMED Instruction Handbook. Medical personnel must be trained in use of ICD-9 books and clinical record coding. Required reference materiel must be available at deployed locations. ACTIONS REQUIRED: CONUS hospitals with taskings to support CONHOSPs UTCs need to ensure an adequate number of personnel assigned to FAC 5140 are trained in procedures for inpatient records. ACTIONS COMPLETED: New training guidance (AFI 41-106) emphasizes training personnel based on their wartime job. Coding of records guidelines (i.e., records to be coded at CONUS depot) exist. This was a procedural item that requires no corrective action. ~ STATUS: Closed.
