          TITLE: Shipment of Medical and Dental Records during
          Mobilization
         
          OBSERVATION: IMA medical records are to be maintained at
          HQ ARPC/SGS according to AER 168-4. A message from HQ
          AFMPC requested medical records be sent to the MTFs. We
          explained dental records would not, because of a special
          XRay, and we would prefer to send the completed AF Form
          1480 in lieu of the medical record. HQ USAF was contacted
          and agreed. The audit and completion of the AF Form 1480
          for activated IMAs was implemented. After implementation,
          HQ ARPC changed this policy, and the medical and dental
          records were sent. These records were pulled and given to
          customer service to ship with the field records.
         
          DISCUSSION: Mobilized IMAs did not necessarily go to the
          base where originally ordered. Some were changed by HQ
          ARPC, others were sent TDY by the base. This made it
          almost impossible to determine where records actually ended
          up. To date, three boxes of records were lost in the mail.
          Now that demobilization is occurring, we have only received
          approximately 200 records and 30-40% of these are missing
          either the medical or dental record.
         
         LESSONS LEARNED: The IMA and the Air Force would be best
         served by leaving the medical and dental records at HQ ARPC
         and sending a completed AF Form 1480. The AF Form 1480
         includes a history of all surgeries, chronic illnesses,
         medications, last physical date, blood type, HIV results,
         dental exam, panograph XRays, bite wing XRays, GYN exam,
         EKG, allergies, and a brief family history. If a MTF
         needed any other information contained in the records, a
         telephone call could answer their question or the
         information could be mailed or FAXed. Now we may never
         receive some of the records, and the IMAs will have to
         reaccomplish medical and dental requirements.
         
         RECOMMENDED ACTION: Send a completed AF Form 1480, as HQ
         USAF originally requested, instead of the medical and
         dental record.
