          4. (U) TITLE: WARTIME MAINTENANCE REPORTING

          5. (U) OBSERVATION: Wartime maintenance reporting requirements
          were not followed.

          6. (U) DISCUSSION: PS Reg 525-1 was developed for deployment
          and subsequent operations. There are checklists, reporting
          formats, and specified unit requirement for the transmission of
          information to the G-4. Most of these aids were not used, or
          when a unit attempted to use them, the staff found the report
          formats to be cumbersome, obtuse, and unnecessary.

          7. (U) LESSON LEARNED: Pre-developed reporting formats are not
          always used because the information needed is situational.
          Report formats are based on assumptions and do not always
          reflect the actual method of business. Report formats should
          be developed based on the situation's requirements.

          8. (U) RECOMMENDED ACTION: FS Reg 525-1 should be revised from
          front to rear. The manual should be simplified.
