    MEMORANDUM TO: Commander, Task Force Evac, 332d Medical Brigade

    THRU: Commander, 14th Preventive Medicine Unit.

    1.  DATE OF SURVEY: 4 February 1991.

    2.  TYPE OF SURVEY: General Field Sanitation Survey.

    3.  UNIT: 410th Evac Hospital.

    4.  DISCREPANCIES:


        LATRINES

          a. No pesticides being used to control flies.
          b. Latrine cans are not being burned out properly. Cans are being
             dumped into a landfill which is subsequently burned.
          C. Latrine cans are being dragged, uncovered, out of the compound.
             Spillage is likely.
          d. No urinal tubes in place.
          e. Inadequate hand washing facilities near latrines.

        MESS FACILITY

          a. Potentially hazardous foods (eggs, cheese, and butter) were not being
             stored at temperature below 45 degrees F.
          b. Some foods (MRE's) were stored outside, without cover.
          C. mess hall soakage pit was too shallow, and contained standing
             liquids. Food particles are not being properly strained from wash
             water, and
             burned.
          d. Wash rag can was not properly chlorinated.

    5.  RECOMMENDATIONS:


        LATRINES

        a.  Place fly bait in latrines, or apply a pesticide with residual killing
            power. Fly bait may be purchased locally at the Al Qusamah Hardware
            Store.
        b.  Burn out latrine can contents in the cans, with I part magas, 3 parts
            diesel. Burn to fine ash.
        C.  If latrine cans must be drug outside compound for burning out, cover
            with trash bags or other suitable material to prevent spillage.
        d.  Construct 3 urinal tubes in the vicinity of each male latrine.
        e.  Place a handwashing facility in the vicinity of each latrine.

        MESS FACILITY

       a. Throw out foods not being stored properly. Store all potentially
           hazardous food in accordance with T8 Med 530.
       b. Move all foods inside or cover.
