SUBJECT: AR 15-6 Investigation, ordnance Explosion, Doha,
Kuwait, 23 July 1991
1. ethis investigation was conducted per appointment at
Enclosure I.

2e' Findings.
-a. At 1420, 23 July 1991, at Doha, Kuwait, unexploded
ordnance being unloaded from a 1 1/2 ton trailer into a pit for
subsequent destruction detonated for unknown reasons, killing
three US Army soldiers instantly-
(1) The most probable scenario is that an M42/M46
submunition, stacked with dozens of others in a cardboard box
without required cushioning and jostled about during transport-1
armed, then detonated as the box was being unloaded from the
trailer, setting off almost simultaneous explosions in the pit
and the trailer. See Exhibit 46 for a detailed narrative. See
Exhibit 44 for a sketch of the area.
(2) The force of the blast killed instantly
The PFC's body stayed
The deceased soldiers' remains were not collected in
accordance with FM 10-j63 because medical personnel rather than
graves registration personnel performed the task. See Exhibits
48, 49, and 50 for comments an4 area clearance from the mortuary
detachment. Certificates of deaths are at Exhibits 42 and 43.

MOD), 111th ordnance Group personnel in performance of their
mission to clear the Doha camp of unexploded, ordnance scattered
over the north and south compounds following an 11 July 1991
explosion. was assigned to the 58th Combat Engineer
Compa.hy (czC), 11th Armored Cavalry Regiment (ACR), and was -
attached to the regimental headquarters engineer section. He was
acting as the track commander of the M113 Armored Personnel
Carrier (Apci that towed the trailer to the disposal pit. The
M113 driver, , also assigned to the 58th CECt
was the fourth soldier in the immediate vicinity of the
