                      (2) Immunization status:

                           a. A new HSC test form titled "Individual Medical
             History" (HSC Test Form 621) provides information on a soldiers
             immunization/chemoprohyliaxis status. During Operation Desert
             Shield this test form, when used in CONUS deployment centers,
             has provided in country medical personnel with valuable
             information. The form should be identified for standard use in
             future deployments. See Encl 4. HSC Form 621-R (Test) (HSHI-QZ)
             1 Aug 90.
                           b.   Existing stock levels and a limited
             industrial production capacity resulted in temporary shortages
             of some prophylactic medications (e.g. immune serum globulin,
             chloroquine phosphate). Actions to provide adequate stock
             levels of projected shortages are being taken. See Encl 1:
             Information Paper, SGPS-PSP, 16 Nov 90, Subject: Preventive
             Medicine Support. Encl 5: Memorandum, SGPS-PSP, 29 Nov 90,

                           C.   Malaria prophylaxis information has been
             provided to the field by WRAIR. The following Malaria risk
             areas have been identified related to Operation Desert Shield:
             Iraq, northern areas; Iran eastern areas; Kuwait may present a
             risk due to recent occupation by Iraqi troops. See Encl 8:
             Message, WRAIR, 21130OZ Nov 90, Subject: Malaria Prophylaxis.

                      (3) Disease and nonbattle Injury: USCINCENT has
             provided instructions for reporting DNBI surveillance data
             (See Encl 9: Message, USCINCENT, 170834Z Nov 90, Subject:
g requirements are
             implemented improved DNBI data will be available. Based on
             reports received for the period 7-27 November'1990 the rate of
             hospital admissions for DNBI was 2.5/1000 soldiers/week.
