9. Separation Physical:
(1) Naval Reservists having a "documented physical
examination" within the preceding four years and no significant
health problems during that interval require for separation only a
SF 600 entry to their health record signed by member pjgg a .'Type 2"
dental screening.
(a) SF 600 entry requirements. Review the interval
medical history with the service member, and have the member sign
and date the following statement: "You have been examined within
the past 12 months and are considered physically qualified for
separation from active duty. No defects have been noted which would
disqualify you from the performance of your duties."
(b) If the member has had a change in their health
or does not have a documented physical examination meeting the
above requirements, then a complete physical examination is required
prior to being released from active duty. This may be started in
country; limited laboratory, X-Ray and specialty consultation
support is available from Fleet Hospital 15 and Foxtrot SSCo/MCH.
Portions that cannot be completed in country must be done by a CONUS
facility prior to separation. Required physical exam procedures
that cannot be completed in SWA should be noted an the SFBB. These
procedures will be completed in CONUS.
(2) Members retained an active duty under stop loss will
require routine RAD physical examinations with "Type 211 dental
examinations per the Manual of the Medical Deparment.
(3) For all physical examinations, ensure completion of
separation physical requirements to the greatest extent possible
prior to redeployment for Camp Lejeune, NC.
(4) Ensure completion of separation dental requirements to
the greatest extent possible prior to redeployment for Camp Lejeune,
NC. Dental examinations are expected to be available through Charlie
Collecting and Clearing Company and will be available through the
Dental Detachment, 2d Dental Battalion, Camp 15.
a. Headquarters Battalion will establish and maintain a
Battalion Aid Station at Camp 15 for transiting 2d Marine Division
Personnel. Dedicated emergency vehicle(s) and immediate access to
telephones are required.
b. Units will ensure appropriate organic medical coverage for
sick call and Echelon I trauma care at Camp 15 and during all phases
of the redeployment. When an Aid Station is established, provisions
for a dedicated stand-by emergency vehicle and communications for
calling additional assistance are the responsibility of the unit.
